    Breed Survey Fee

    An Application for Survey Form (GSDCA BS1) may be downloaded from the GSDCA website. Alternatively contact the Victorian State Breed Survey Registrar Gillian Cafari on 03 5420 7385. The completed form together with the relevant paperwork (and fee) must be received by the registrar at least 7 days prior to the Survey date.

    To apply for Breed Survey you must provide the following with your fee payment here:

    1. An Application for Survey Form (GSDCA BS1)
    2. A copy of the ANKC registration certificate (blue form) in the name of the owner. Note: animals on the limited register cannot be accepted for Survey.
    3. Copies of ‘A’ and ‘Z’ Stamp results/certificate.
    4. Five generation pedigree.
    5. For all imported animals a copy of the Title Verification Certificate as received from the GSDCA Title Verification Registrar.
    6. For previously presented animals that have failed survey, a copy of the GSDCA Form BS2-Notification of Failure must be supplied.

    Hip & Elbow Contract

    This payment is for a GSDCA Hip and Elbow Contract, for current financial members of the GSDCV. 

    Your payment includes a postage and handling fee of $2.00

    1. A copy of your dog’s ANKC pedigree is required to be submitted (attached below) at the time of purchasing the contract.

    ZAP Assessment Fee

    A ZAP Application Form (GSDCA Z1) may be downloaded from the GSDCA website. Alternatively contact the Victorian Registrar Gillian Cafari on 03 5420 7385. The completed form together with the relevant paperwork (and fee) must be received by the registrar at least 7 days prior to the Test date. Open to pedigreed GSD’s over 9 months and under 8 years

    To apply for ZAP Assessment you must provide the following with your fee payment here:

    1. An Application for ZAP Assessment Form (GSDCA Z1)
    2. A copy of the ANKC registration certificate (blue form) in the name of the owner. Note: animals on the limited register cannot be accepted for Assessment.
